OVERVIEW
OF THE POSITION
Responsible
for directing and assisting in the care and treatment of residents, including
life skills training and limited psychosocial interventions, also may act as
shift lead in the absence of the supervisor. Performs a wide variety of
paraprofessional duties to assist in delivering services to the mentally ill,
including but not limited to; facilitating resident cooking, cleaning and
general maintenance duties. May be assigned in certain settings, to administer
medications and perform basic nursing duties under the supervision of the
Registered Nurses (Division 47 of the Nurse Practice Act.)
RESPONSIBILITIES/DUTIES
¨
Determines
and provides basic case management services to residents, to include banking,
shopping, cooking, cleaning, laundry, mental and physical health provider
appointments, and assists residents to interface with other social services.
¨
May
assume lead position in the absence of or at the discretion of the
Administrator and/or Program Manager.
¨
Participates
with the health care team in selecting appropriate candidates for residence.
¨
Provides
discharge planning.
¨
Admits
and discharges residents.
¨
Administers
medications and performs basic nursing duties as prescribed by the physician
and assigned by the Registered Nurse according to OAR 851-047-000.
¨
Participates
in Residential Service Planning, acts as a member of the Service Team.
¨
Provides
therapeutic interventions with residents as prescribed by the Residential
Service Team.
¨
Assures
that staff scheduling is adequate, may act as scheduling lead.
¨
Observes
and monitors residents, charts resident behaviors, activities and changes in
mental and physical conditions.
¨
Interacts
with other members of the health care team.
¨
Directs
and participates in facility cooking, cleaning and general maintenance duties.
¨
Directs
staff and assists residents in activities of daily living, to include bathing,
grooming, hygiene, laundry, cooking, cleaning and general health.
¨
Plans,
coordinates and supervises recreation activities and outings of residents.
¨
Transports
and supervises residents as necessary.
¨
Monitors
safety and security of environment.
Leads and assists in management of behavioral emergencies.
¨
Audits
resident charts, enters information into computer, and answers telephones.
¨
Informs
Administrator and Program Manager of resident and facility needs and concerns.
¨
Maintains
positive work relationships with member of Phoenix Place staff, medical and
rehabilitation staff.
¨
Attends
and participates in general staff meetings.
¨
Maintains
timely, complete written records of resident’s contacts, attendance and other
documentation required by state and county regulations for residential care.
¨
Performs
other duties as assigned that are consistent with level of experience and
preparation.

obtain P.A.R.T. Training Certificate.
EDUCATIONAL/VOCATIONAL
PREPARATION
¨
High
School Diploma or its equivalent, and
¨
Four
years experience working in the mental health field, or
¨
Completion
of one year mental health tech training or its equivalent and two years
experience working in the mental health field, or
¨
Other
comparative combination of education and experience demonstrating the
knowledge, skills and ability to perform the above-described duties.
COMPENSATION
This is a part-time/on-call position
with the Mental Health department. The
starting wage for this position is $14.49 per hour. Future increases are
reviewed annually and are based upon performance and movement on a 7 step scale
which extends up to a current maximum of $18.85 per hour. This position is classified as Non-Exempt by
the Fair Labor Standards Act (FLSA) and, therefore, is eligible for overtime
pay.
