Birth & Death Records

Klamath County Department of Public Health, Vital Records

Records of all births and deaths occurring in Klamath County are available at the Vital Records Reception Window at the Health Department from 8am to 5pm, Monday to Friday, except for holidays.

Cost of Birth and Death Records

Single copy - $20.00

Extra copies - $15.00 (same record, ordered and paid for at the same time)

Payments accepted:

Cash

Checks payable to: KCHD or Klamath County Health Department

Credit cards: Visa and MasterCard

You need to provide / bring the following information:

Birth Certificate Information:

Name of child (First, Middle, Last

Birth date of child

Mother's maiden name

Father's full name (if it appears on the record)

Picture ID bearing the same name as that of child, appearing on the certificate

Death Certificate Information:

Full name of deceased (First, Middle, Last)

Date of death

Place of death

Picture ID showing blood relationship

Record Pick-up

Certified records will be ready for collection 24-hours after payment

Records are available for the first 6 months after the birth or death, after which time certificates can be obtained from the Vital Records Office in Portland.
