What is CaCoon?
CaCoon
is a program for families that have children with special health
needs.
As its name suggests, CaCoon (CAre COordinatiON)
provides support for
families:

Helping parents find resources |
 |
Helping coordinate services |
CaCoon
recognizes the importance of the family. Because you and other family
members
have a central role in the care of your child, all services are planned
around
the desires and concerns of your total family.
How can
CaCoon help?
Through CaCoon, you can receive help from a
trained local care coordinator,
who can:
 |
Screen children who are at risk and monitor their progress |
 |
Identify developmental problems early |
 |
Provide medical and legal information about your child's condition |
 |
Find important care services as close to your home as possible |
 |
Teach special techniques for caring for your child |
 |
Give support in times of stress and crisis |
CaCoon's goal is to help you and your family learn new skills to become
as independent as possible
in caring for your child. Who
can use CaCoon?
CaCoon serves families with children through
birth-21 who have complex
conditions which result in special medical,
educational, vocational and social
needs. Services may relate to:

cleft lip and/or palate |
 |
cerebral palsy |
 |
spina bifida |
 |
hydrocephalus |
 |
heart disease |
 |
rheumatoid arthritis |
 |
chronic orthopedic problems |
 | metabolic disorders |
 |
genetic disorders |
 |
spinal cord injuries |
 |
chronic lung disease |
 |
motor dysfunction requiring
special feeding programs |
 |
other chronic conditions |
For
children age six and over (who are not eligible for CaCoon), the
Child
Development and Rehabilitation Center (CDRC)
provides comprehensive
services through special clinics around the state and limited financial
assistance for approved medical care.

CaCoon
assures that your family, as well as your child, will receive the
best service possible.
CaCoon
-- a cooperative effort between families and health care
providers.
Raising a child with special health care needs can
be challenging and rewarding.
It also
can be confusing and exhausting -- for the whole family. You and your
family
may need the services of many different
professionals and agencies. You may
have to
travel away from home for specialized health care. Sometimes you just
may
need to talk to someone who has been in your shoes and understand what
you are
going through.

A special program for your
children
Extra Care
and Attention
Your newborn is special, delicate and
precious. That's why we've created
"Babies First!" So you and your baby
get extra attention you can't get anywhere
else.
A public health nurse will visit you
regularly - at home. She'll talk to you about your
baby, check its progress and suggest
new ways you can encourage your baby's
growth and development.
There's no charge for "Babies First!"
All you have to do is agree to participate.
Regular
Home Visits
A public health nurse may visit you
regularly
until your baby is three years old.
She'll
give your baby health and
developmental
screenings. She will work closely
with your
doctor or clinic. Your baby's health
will always
be in very good hands
Expect
a "Babies First!" visit:

Shortly after your baby's birth |
 |
At 4 months |
 |
At 8 months |
 |
At 12 months |
 |
At 18 months |
 |
At 24 months |
 |
At 36 months |
Home visits may be
done at other times as well, depending on the needs of the family.
Personal
Support and Encouragement
But "Babies First!" offers still
more. Your public health nurse can teach you games
and exercises that will encourage
your baby's growth and development. She can
answer your health questions or refer
you to the appropriate agency for specialized
attention. You'll learn how to better
care for and understand your baby. Right in your
own home. But you control the program
-- services may be discontinued at any time.
